A. Complete HRA Questionnaire
 
B. Complete Biometric Screening with 85% participation (insured employees)

C. Group Level - offer three onsite "wellness" seminars per year
 
     i. Seminar topics can be: 
 
          1. Nutrition
 
          2. Fitness
 
          3. Mental Wellness 
 
          4. Self-care
 
     ii. Information reported about seminar: 
 
          1. Date of seminar
 
          2. Presenter information
 
          3. Summary of presentation
 
          4. Participant list
 
     iii. Seminars can be offered through: 
 
          1. Wellness Partner
 
          2. Local resources
 
D. Health Coaching
 
     i. Participants qualify for Coaching if they have 4 or more health risks
 
All seminar information must be turned in by October 1, 2015. If a third seminar is scheduled for 4th quarter, please turn in the scheduled seminar information on October 1, 2015 as well. 
 
Incentive: 5% premium discount awarded at the group level if all group requirements are met.
